What is UDID Card / Disability Certificate?
- Unique Disability ID for Persons with Disabilities (PwDs).
- Single document for identification and verification of disability
- A centralized online platform for certification ensuring nationwide uniformity.
- Valid pan-India for availing various government benefits and schemes.
Who can apply?
- Any citizen of India suffering from any of the 21 disabilities recognized under the RPwD Act, 2016
- A legal guardian may apply for a minor or a person unable to apply himself/herself due to the disability
- Those willing to digitize old paper-based disability certificate to a digital one
How to apply and where to apply?
- Where: Apply online directly via the UDID portal (swavlambancard.gov.in).
- How: Register online and complete the application form
- Required Documents: Passport photo, Signature/Thumbprint, Identity Proof, and relevant medical records.
- Offline Alternative: Visit the designated Hospital for assistance.
- Fees: NIL
- Delivery of card : free of cost by speed post at residential address
How to know my application status?
- Visit the official UDID web portal (swavlambancard.gov.in).
- Click on the “Track Your Application Status” tab.
- Enter your either Enrollment Number, or UDID Number, or Aadhaar Number, or registered Mobile Number.
- View the real-time processing status of your application online.
- The AI-powered Swavlamban Card Chatbot available on the UDID Portal can also be used to track the status of UDID applications and to get assistance with queries related to UDID Cards and Disability Certificates
Role of Hospital, PwD, and State
- Role of PwD: Register on the portal, provide accurate information, and contact your chosen hospital to seek date of medical assessment of disability from UDID cell in the hospital. Attend the scheduled medical assessment.
- Role of Hospital: Process applications, verify documents, Conduct disability assessment as per notified Disability Assessment Guidelines, and issue the certificate via the online system.
- Role of State: Onboard authorized hospitals, appoint State Coordinators, and monitor implementation at the district level, Coordinate with Health Departments and other concerned authorities.
